The NDIA can provide funding for support coordination. Whether you are self-managed, plan-managed or NDIA-managed has no effect on the decision to fund you for support coordination. Funding will only be provided if the NDIA decides that it is reasonable and necessary.
Support coordinators can be found in the Provider Finder tool the NDIS my place portal. Your Local Area Coordinator (LAC), Planner or early childhood partner can help you find one and then help by providing plan information to them so that they can understand your plan and assist you in pursuing your goals.
Three different types of support coordination exist: Level 1, 2 and 3.
The Innovative Supports’ support coordinators can assist with your NDIS plan at 3 different levels:
Support Connection is a basic support level (Level 1) for you to understand and implement your NDIS plan to achieve your NDIS goals. Our Support Coordinator can assist you to understand the NDIS system and your funding, connect with a broad range of services, maintain or change services, assist with resolving provider or service disputes, and answer NDIS related questions so that you can achieve the most out of your NDIS plan. Support Connection funding is to help you become independent and become confident with NDIS processes.
Coordination of Supports is to strengthen your own ability to coordinate formal and informal supports and services. Our support coordinator will help you to understand your funding, find suitable services, and link with broader systems of support across a complex service delivery environment.
Our support coordinator will help you to ‘direct your life’ (not just services), help build, maintain and design your support network, your capacity and resilience going forward.
Specialist Support Coordination for immediate, intensive plan implementation for highly complex situations and risks. Our Specialist Support Coordinator for a short time-duration can provide you with immediate intensive assistance to address, reduce and overcome complex barriers impacting your ability to access supports.
Our Specialist Support Coordinator can deal with crises, resolve issues; and is skilled and experienced to negotiate different support solutions in your complex environment to achieve, implement and monitor a complex service plan designed for you.
Our Specialist Support Coordinator will monitor this service plan and focus to increase your capacity and resilience to deal with issues so that you can become more independent going forward and not requiring such intensive help in the longer term.
You as a participant have the right to be involved when the evaluation of your situation occurs and can be involved as well when what supports are required for you. These supports will be the ones that are put in place to assist you to identify with us the supports required to prevent or respond to any crisis that you may be going through or any breakdown of support arrangements that you may be experiencing. It is important also that you have your say so that the your safety and the safety of others can be promoted.
You may change providers as you have choice and control. Firstly let your provider know you wish to change the agreement or terminate agreement with notice period. You may report any issues to the NDIA commission:
